You must have the following forms to participate.

1) Physical Form filled out by doctor

2) Consent to Treat signed by parent/guardian

3) Copy of Insurance Card

4) Rules and Regulations signed.

These forms need to be turned in to Coach Cheatam

Practice: Shorts, t-shirt, wrestling shoes, headgear.
